Learning from Canadians living with T1D to build a digital patient engagement network

Diabetes Action Canada is working with Canadians living with Type 1 Diabetes (T1D) to create a network that will connect patients, accelerate research, and improve how the health system treats T1D. To ensure that this network creates meaningful value for patients, we need your help.

Step 1 – Send out this short survey (approx. 3 mins) to patients in your practice.

Step 2 – Based on the response to this survey, we will select a diverse range of participants to have more in-depth conversations with. These 1-2 hour conversations could be 1-on-1 or in a group setting. They will enable you to articulate the challenges experience by those living with T1D and to shape solutions that make living with T1D easier.

Those selected, we will reach out via email to schedule a time in April.

We will be hosting these conversations both in-person at Toronto General Hospital and through video conference. Participants will be paid for their time.

Please complete this survey no later than end-of-day April 12th.
