Research-to-Action Fellowship and Patient Circle Application 2026
Thank you for your interest in applying to Diabetes Action Canada’s Research-to-Action Fellowship and/or Patient Circles.
Both opportunities are paid and value the time, leadership, and expertise of people with lived, loved, learned, and laboured experience of diabetes. You are welcome to apply for the Fellowship, a Patient Circle, or both. However, applicants can only be selected for one opportunity in the same year. This helps protect participant time and ensures that everyone selected can participate meaningfully without being overextended. If you apply to both, your application will be reviewed for both pathways. The Diabetes Action Canada team will consider your interests, availability, experience, and program fit when making final selections.
You can answer in point form, full sentences, or the format that feels clearest to you. Suggested word counts are included for longer questions, but they are only meant to guide you.

Your responses will be reviewed by the Diabetes Action Canada team and/or members of the selection committee for the purpose of selecting participants and building balanced, inclusive groups. Some questions help us understand eligibility, access needs, cohort composition, and potential conflicts of interest. They are not meant to judge the value of your experience. We will only use your information for this application process unless you give us permission otherwise. Payment details such as banking information, mailing address, or SIN will not be collected through this application form. Selected participants will receive secure payment instructions later.
