About the Hub
The Diabetes Action Community Hub was built by people with diabetes, for people with diabetes and those that support them. Developed in collaboration with our Patient Advisory Committee (PAC)—who live with diabetes every day—this resource connects the community with support networks, peer groups, and resources across Canada.

Facing diabetes can be challenging, and no one should have to do it alone. The Diabetes Action Community Hub was co-designed with input from people just like you. It’s a community-built space where you can find peer groups, resources for life stages, and communities that meet your unique needs. Our mission? To connect, empower, and support the diabetes community.

Submission Guidelines & Review Criteria
Submissions to the Hub will be reviewed monthly and are evaluated based on:
- Alignment with our mission to support, empower, or build community for people with diabetes.
- Relevance to the Hub’s mission.
- Accuracy and completeness of the information provided.
Advisory Group Structure
The Advisory Group plays a key role in the ongoing review and updates to the Diabetes Action Community Hub, ensuring it stays relevant, accurate, and reflective of the community’s needs. This group brings together a range of perspectives and expertise, including:
- Lived – Child: Individuals with firsthand experience as children living with Type 1 diabetes.
- Lived – Adolescent: Adolescents who manage their diabetes, providing insights into youth-focused challenges.
- Lived – Adult: Adults living with Type 1 diabetes, bringing perspectives on managing diabetes in adulthood.
- Loved – Caregivers, Parents: Family members or caregivers who support individuals with Type 1 diabetes.
- Laboured – People who work in diabetes: Healthcare providers, diabetes educators, community health workers, and other professionals supporting diabetes care.
- Learned – Researching diabetes: Researchers and academics focused on advancing knowledge and treatment options in Type 1 diabetes.
