Access Alliance
Access Alliance is a Community Health Centre, which has been serving Toronto’s marginalized, newcomer and immigrant communities for thirty-five years. They are focused on addressing the deepening disparities in society and leveraging the power of collaboration through systems change initiatives.

South Riverdale Community Health
South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto.

Diabetes Canada
Improving the quality of life for people with diabetes is at the heart of Diabetes Canada’s commitment to all Canadians diagnosed with this chronic health condition. That means connecting children and youth with type 1 diabetes through Diabetes Canada’s D-Camps; providing healthcare professionals and people with diabetes with expert information that improves their care and self-management; assisting researchers working on leading-edge treatments; and advocating on behalf of those with diabetes for equity, improved access to supplies and more.

Research Canada
Research Canada is a national, broad-based alliance dedicated to advancing health research and health innovation through collaborative advocacy. Their mission is to improve the health and prosperity of all Canadians by championing Canada’s global leadership in health research and innovation.

CanScreen T1D
Led by Dr. Diane Wherrett from The Hospital for Sick Children, CanScreen T1D is building a robust network to support clinical follow-up, intervention trials, and metabolic monitoring for individuals at risk of T1D. Through this partnership, DAC and CanScreen T1D will streamline processes, reducing duplication of effort and enhancing the impact of screening and follow-up care.

Indigenous Diabetes Health Circle
Rooted in Indigenous cultural principles, this partnership emphasizes the importance of connection—between people, lands, waters, knowledge, and spirit. It honours the teachings of wholistic wellness, and upholds core values such as reciprocity, representation, and relevance. Together, we will partners to continue to support Indigenous-led programs, advance culturally grounded education, and uphold Indigenous data sovereignty and self-determination in all aspects of the work.
